Welcome patients! We are pleased you have chosen Whole Woman Health. When you make your appointment with us, you will receive an email with links to our new patient forms.  Please check your email for emails from us.  You may also click the links below to access the forms after you make an appointment.

We encourage you to explore our website, as most patient questions are addressed in the contents.  If you haven’t already, please call our office at 515-243-6309 to schedule your appointment. After you have scheduled with our office staff, you will receive an email with a link to the new patient paperwork. We will need the paperwork submitted to us before your initial consult, and we ask that you arrive 10 minutes prior to your scheduled appointment time. This helps the check-in process run more efficiently so you are able to begin your appointment on time.

Whole Woman Health makes every attempt to provide you with the quality medical care you expect and deserve. If you have any questions or concerns, please contact our office staff at 515-243-6309. Patients are seen by appointment only and we recommend you schedule as far in advance as possible, as we are typically scheduling 4-6 weeks out. If you must cancel an appointment, we ask that you contact us more than 48 hours in advance so we are able to fill the spot. The patients on our lengthy cancellation list will appreciate your consideration. We send reminder emails or texts (depending on your preference) a week in advance so you are able to give sufficient notice of a cancellation or reschedule request. Please notify us if you will need accommodations for a disability.

*Due to fragrance sensitivity, we ask that you refrain from wearing perfumes or scented body sprays or lotions at the time of your visit.

Monday, Wednesday & Thursday:  9:00am-5:00pm (closed noon-1:00 pm for lunch)
Friday: 9:00am-3:00pm
Tuesday:  Closed (appointments may be arranged for special circumstances)

From I-235:

  1. Take the 42nd St Exit (Exit 5B)
  2. Travel North on 42nd St until you come to a T intersection. This will be Forest Ave.
  3. Take a left on Forest Ave, and travel 3 blocks to our location at 4507 Forest Ave (we are on the corner of 45th & Forest on the north side of the street)

From I-80:

  1. Take the Merle Hay Rd Exit (Exit 131)
  2. Travel south on Merle Hay Rd to Hickman Rd and turn left
  3. Take Hickman Rd to 48th St and turn right
  4. Take 48th St to Forest Ave and turn left
  5. Travel 3 blocks to 4507 Forest Ave (we are on the corner of 45th & Forest on the north side of the street)
